CCD / 1.9 disklabel manpage update patch

So, since vinum was not working, I tried to create a CCD,
and recalling the discussion on the lists, remembered someone talking
about this..

It looks like the needed FS_CCD disklabel changes didn't make it to the
docs..

attached is a patch for ccd(4) and ccdconfig(8).

not sure what else is lurking in the docs related to the disklabel
changes...
