It looks like s_ttyvp has been vgone_vxlocked() as it is set to VBAD
and has v_opencount==0.

I don't know if the bug comes from this call to vgone_vxlocked().  If
not, we could lazily check for VBAD in ctty*() and reset s_ttyvp if it
is BAD.
